Nebraska Pumps Backflow Protection

The discharge piping from any pump and pumping equipment must be equipped with a backflow preventer. Must be placed before any other device or branches in distribution piping. Check valves must not be buried at the well for backflow prevention. Device must be located w/in 1 ft of discharge head and prior to any other devices.

Nebraska Storage tanks Storage Tank

Must be approved for potable use. Pressurized tanks must be equipped with identification including size, max working pressure, and name of manuf. Non-pressurized (cisterns) must be constructed of material with structure to adequately withstand burial.

Montana Seals Sealing Material

Bentonite water slurry shall contain no less than 1.5 pounds of bentonite per gallon of freshwater. The mixed slurry shall weigh no less than nine pounds per gallon.

Montana Grout Neat Cement

Not more than six gallons of clear water per 94-pound bag of Portland cement. Up to five percent, by weight, of bentonite clay may be used to improve flow and reduce shrinkage. No sand or gravel is to be used in cement grout.

Montana Grout Cement

Cement grout used for well abandonment shall conform to requirements of ARM.

Consisting of not less than one-half pound of commercial bentonite clay to one gallon of clear water.

Montana Casing Plastic Casing Joints

Shall be watertight. Either "bell" type or coupling hubs or threaded couplings are approved. Hub couplings shall be of material meeting the specifications for plastic casings as stipulated in ARM 36.21.645. Solvent cemented joints shall be made in accordance with manufacturer's directions.

Montana Casing Inner Casing

All inner casing shall be of steel, in new or like new condition, being free of pits or breaks; or shall be of PVC conforming with ASTM F480-81 per ARM 36.21.645. Inner casing installed in a well shall extend or telescope at least 4 feet into the lower end of the well casing. In the event that more than one string of inner casing is installed, each string shall extend or telescope at least 4 feet into the adjacent larger diameter inner casing. In the event inner casing must be driven, it must meet the specifications of ARM 36.21.640.

Montana Construction Artificial Gravel Packed Wells

Annular space to this depth to be filled w/ grout. If formation of low permeability is not encountered, upper drill hole & unperforated casing to extend min of 18 ft. If temporary casing is to maintain drill hole, annular space to be kept full w/ cement grout or bentonite clay. Injection pipe to be capped w/ watertight seal or plug. May extend into other formation of lower permeability overlying the water-bearing zone. Suitable bridge to be installed in annular space between gravel pack & sealing material. Upper diameter of drill hole at least 3 in. > O.D. of production casing to extend from surface into clay. Upper drill hole & unperforated production casing to extend to min depth of 18 ft. below land surface. Montana Construction Consolidated Formations

Annular space between rock formation & permanent casing to be sealed w/ cement grout. annular space between upper drill hole & permanent casing to be kept at least one half full w/ bentonite slurry. Lower part of casing to be sealed into rock formation w/ cement grout. Unperforated permanent casing to extend to at least 3 ft. From surface into sound consolidated formation. Upper drill hole of at least 3 in. > in diameter than nominal size of casing to extend to at least 3 ft. into solid rock.

Montana Construction Flowing Wells

Unperforated well casing shall extend into the confining stratum overlying the artesian zone. The casing shall be adequately sealed into the confining stratum so as to prevent surface and subsurface leakage from the artesian zone. If the well flows at land surface, it shall be equipped with a control valve so that the flow can be completely stopped. The well shall be completed with packers or appropriate sealing material that will eliminate leakage around the well casing.

Montana Construction Unconsolidated Formations W/ Clay Beds

Well casing may be terminated in clay strata (at least 6 ft thick) if casing is sealed in same manner as required of consolidated strata.

Montana Construction Unconsolidated Formations W/O Clay Beds

An unperforated well casing shall extend to at least one foot below the known seasonal low water table. An upper drill hole having a diameter at least three inches greater than the nominal size of the permanent casing shall extend to at least 25 feet below land surface.

Mississippi Testing Testing - General

Pumping test, elog and gamma ray required, physical/chemical water test, bacteria test. Must be free of coliform bacteria following disinfection.

Mississippi Licensing Licensing - General

All water wells shall be constructed by a licensed water well contractor unless specifically exempted by statute.

Mississippi Grout Grout - General

Either neat cement, cement grout, cement-bentonite mixture (5-8% bentonite) or bentonite. Bentonite pellets or bentonite chips may be added under free-fall conditions for depths not exceeding 25 ft. Free-fall addition of any other type grout from the surface is prohibited. Granulated or pelletized bentonite may be placed to greater depths only if introduced through a tremie pipe. For continuous lengths of grout not separated by multiple screens, grout shall be introduced in one continuous operation from the top of the water bearing stratum to the ground surface. Outer casing shall be grouted from at least 10 ft below top of casing at the underground discharge head, or pitless adapter, to top of casing.

Mississippi Gravel Gravel - General

Under reamed and sized according to sand and screen. Shall be disinfected with chlorine residual of 50 ppm unless is clean, pre-packaged gravel.

Mississippi Disinfection Disinfection - General

Residual of no < 5ppm of chlorine to be maintained in water used for development. Water used in drilling & construction to meet Safe Drinking Water Act Regulations. Gravel, well & adjacent aquifer to be disinfected using soln of 50 mg/L free chlorine for 24 hrs. Well then to be pumped until 2 consecutive chlorine-free samples are collect that show no coliform bacteria. Procedure shall meet AWWA Standard.

Mississippi Check Valves Check Valve - General

All wells shall have a check valve installed in the discharge line. All new unmetered wells larger than 6 inches in diameter shall have a minimum of 4 ft of straight pipe installed between the casing head and the check valve, and all other equipment connected on the discharge side of the check valve.

Mississippi Casing Steel

To meet latest revision of AWWA standard.

Mississippi Casing PVC

If SDR 17, depth to be 500 ft. If SDR 21, depth to be 250 ft. If SDR 26, depth to be 125 ft. May be allowed if justification for its use outweighs its risk of failure. AWWA Standards.

Mississippi Casing Casing - General

AWWA Standards All casing shall terminate not less than 12 inches above ground. If not above 100-yr flood elevation, well shall be fully sealed with a Braden Casing Head or approved equivalent. Metal casing required if waters contain chloride concentration in excess of 250 mg/L and/or TDS exceeding 1000 mg/L, and must only use Halliburton grouting method to grout thoroughly the annular space from bottom of casing to ground surface.

Mississippi Caps Caps - General

If the casing is not covered with a recorder box or pump housing, then it must have a locked, overlapping cover or other suitable cover capable of preventing unauthorized access to the casing head and access plug.

Mississippi Annular Space Annular Space - General

Grouted from 10 ft below surface to the surface. Exceptions: wells located within 1/4 mile of known area of contaminated aquifer shall be grouted from top of water-bearing stratum to ground surface. Wells located within 100 ft of potential source of pollution such as sewers, septic tanks, and landfills shall be grouted from the lowest level of disturbed earth immediately adjacent to the casing to the top of the water bearing stratum.

Mississippi Additional Requirements Additional Requirements - General

Apply to wells that penetrate water bearing strata or are greater than 25 ft in depth. Licensed well contractor to decommission well. If less than 25 ft depth can be plugged by other than licensed contractor. Department of Environmental Quality requirement: Pressure grout from bottom to top. See Decommissioning Procedures approved by DEQ.

Missouri Location Subsurface Disposal Field

Missouri Check Valves Check Valve - General

If a check valve is installed in a buried water line between the well casing and the pressure tank, the water line between the well casing and the check valve shall meet the requirements for a suction line unless equipped with an air release valve

Missouri Annular Space Annular Space - General

Top 20 ft and bottom 30 ft of annulus must be grouted.

Minnesota Location Waste Stabilization Pond

300 ft from a municipal or industrial wastewater rapid infiltration basin, a municipal wastewater stabilization pond with 500 or more gallons/acre/day of leakage, and a liquid manure storage basin or lagoon that is unpermitted or noncertified according to chapter 7020; 150 ft from a municipal wastewater stabilization pond with less than 500 gallons/acre/day leakage, an industrial wastewater stabilization pond, and a liquid manure storage basin or lagoon that does not have a concrete or composite liner, but has an earthen liner that was constructed under a Minnesota Pollution Control Agency permit or is certified according to chapter 7020, 100 ft from a liquid manure storage basin or lagoon with a concrete or composite liner in accordance with chapter 7020. Distances are doubled to 600, 300 and 200 ft from a sensitive well. A "Sensitive water-supply well" is defined as a water-supply well with less than 50 feet of watertight casing where the casing does not penetrate a confining layer or multiple layers of confining materials with an aggregate thickness of 10 feet or more.

300 ft from absorption area of a soil dispersal system with an average design flow >= 10,000 gallons per day. 150 ft for systems <10,000 gal/day serving facilities handling infectious or pathological wastes, 50 ft for other systems < 10,000 gal/day. Distances are doubled to 600, 300 and 100 ft from a sensitive well. A "Sensitive water-supply well" is defined as a water-supply well with less than 50 feet of watertight casing where the casing does not penetrate a confining layer or multiple layers of confining materials with an aggregate thickness of ten feet or more.

Minnesota Grout Cement-Sand Grout

A fluid mixture of Portland cement, sand, and water in the proportion of 94 pounds of Portland cement, not more than 1.0 cubic foot of dry sand, and not more than six gallons of water. Admixtures to reduce permeability or control setting time must meet ASTM Standard C494/C494M-04.

Minnesota Casing Stainless Steel

Stainless steel casing is not allowed for potable wells but only monitoring wells and environmental boreholes.

Minnesota Casing Driven

In unconsolidated formation, bentonite grout, bentonite powder, or granular bentonite must be maintained around outside of casing. May only be driven through A. unconsolidated formation, B. sandstone bedrock including the St. Peter, Jordan, Franconia, Ironton-Galesville, Mt. Simon, Hinckley, or Fond du Lac formations; C. ten feet or less of limestone or dolomite bedrock including the Cedar Valley through Galena groups, the Platteville formation, or the Prairie du Chien group; D. ten feet or less of the St. Lawrence or Eau Claire confining layers; and E. two feet or less of the Decorah or Glenwood confining layers.

Minnesota Casing Drive Shoe

A drive shoe is required whenever driving casing. Must be A) made of steel or iron with hardened, beveled cutting edge B) have a wall thickness equal to or larger than the casing thickness and C) be threaded or welded to the bottom of the casing.

Minnesota Pumps Pumps - General

Must not use pump that requires priming for ordinary use.

Minnesota Construction Limestone or Dolomite

Must not be completed in limestone or dolomite bedrock unless overlain by at least 50 ft of unconsolidated material, sandstone or shale extending in all directions for minimum radius of 1 mile. If completed in limestone or dolomite and static water level is more than 10 ft above limestone/dolomite, steel casing must be installed into limestone/dolomite by driving casing with drive shoe through unconsolidated materials or sandstone not more than 10 ft into limestone/dolomite. If static water level is less than 10 ft above limestone/dolomite bore hole must be drilled a minimum of 3.0 inches larger, or 3.5 inches larger for casings deeper than 100 ft and larger than 12 inches inside diameter, than the outside diameter of the casing or couplings, whichever is larger. The bore hole must extend a minimum of 20 feet below the static water level. Steel casing must be installed to bottom of borehole.

Minnesota Construction Construction - Water

Water used for drilling, development, hydrofracturing, sealing, repair, or rehabilitation, if not from the well itself must come from a potable source, contain free chlorine residual at all times, and be conveyed and stored in clean, sanitary tanks and water lines.

Arkansas Construction Construction - General

If borehole extends more than 10 ft below bottom of screen, must be filled with grout from bottom of bore hole to within 10 ft or less of screen. Flowing well must be constructed with flow control and have outlet screened with noncorrosive mesh with openings of 1/16 inch or less. A casing that extends more than ten feet into limestone or dolomite bedrock must be installed in a bore hole a minimum of 3.0 inches larger, or 3.5 inches larger for casings deeper than 100 feet and larger than 12 inches inside diameter, than the outer diameter of the casing or couplings, whichever is larger. Must be constructed to prevent entry of flood water. Materials used in construction of a potable water-supply well that contact water must not exceed 8% lead except that solders and flux must not contain more than 0.2 % lead.

Minnesota Construction Construction - Dug Well

May only be constructed in unconsolidated formation. Junction of cover with well casing must be made with watertight gasket and must provide a well vent. Pump opening and connection below established ground surface must be made watertight according to part 4725.4850 subpart 1, or with concrete or cement. Unless grouted from surface to depth of 50 ft or through a confining layer, must be located according to part 4725.4450 subpart 2.

Minnesota Construction Construction - Drilling Fluid

Additives must meet ANSI/NSF 60-2003e. Bentonite drilling fluids used to construct or repair a water-supply well must have a measurable chlorine residual at all times during drilling or repair, except for a remedial well where chlorine will interfere with water quality analysis or remediation.

Minnesota Chemical Treatment Chemical Treatment - General

Chemicals used to remove or treat contaminants, or to rehabilitate wells must meet ANSI/NSF Standard 60-2003e. Sodium or calcium hypochlorite may be used if registered by the USEPA according to the Federal Insecticide, Fungicide, and Rodenticide Act (FIFRA), section 3(c)(7)(A), as an antimicrobial pesticide for use in potable water. Chemicals must be neutralized or removed from well, boring, piping systems prior to use.

Minnesota Annular Space Annular Space - General

The annular space between an inner casing and an outer casing must be grouted for its entire length by pumping neat-cement grout or cement-sand grout through a tremie pipe or through the casing as specified in part 4725.3050.

Minnesota Aquifer Interconnections Aquifer Interconnections

Must not be constructed to interconnect aquifers separated by a confining layer or interconnect an unconsolidated aquifer and bedrock aquifer. Must not have bore hole, gravel pack, or screen extending through more than 1) 10 ft of confining layer, except for Decorah or Glenwood formations 2) 2 ft of Decorah or Glenwood formations and 3) 50% of confining layer.

Minnesota Additional Requirements Additional Requirements - General

Must not be used for disposal or injection of surface water, groundwater or any other liquid, gas, or chemical. May be used for injection of water to conduct slug test if injected water was originally taken from that well or is potable.

Arkansas Grout Cement-Sand Grout

Mixture of Portland cement, sand, and water in the proportion of 94 lbs of Portland cement, not more than 1.0 cubic ft of dry sand, and not more than six gallons of water. Admixtures to reduce permeability or control setting time must meet ASTM Standard C494/C494M-04. Portland Cement should meet ASTM Standard C150-04a.

Minimum isolation distance 10 ft, or 5 ft if 1) the person constructing the well or boring, or the person installing the pipe, line, or tank, marks the well or boring with a permanent sign warning of the location of the electric line, liquid propane tank, or gas pipe; and (2) during construction or sealing of the well or boring:
(a) the electric line has been de-energized and visibly grounded, or insulating
barriers not a part of, or an attachment to, the equipment or machinery have been erected to prevent physical contact with the line during well or boring construction; and
(b) the propane tank does not contain flammable or volatile gas.

100 ft from tanks, vessels, or components of a wastewater treatment unit. 50 ft from sand filter, peat filter, or constructed wetland. 50 ft for water treatment backwash holding basin, reclaim basin, or surge tank with direct sewer connection. 20 ft from water treatment backwash holding basin, reclaim basin, or surge tank, and associated piping, with a backflow protected sewer connection.

Minnesota Pump Discharge Lines Pump Discharge Lines - General

If check valve installed in a buried water line between casing and pressure tank, water line must meet requirements of part 4725.5150 unless equipped with vacuum release or combination air release and vacuum release located between the check valve and well. Pump discharge lines must be constructed of materials approved in part 4725.5150 subpart 1.

Michigan Construction Construction - Flowing Artesian Well

Shall be grouted to protect the artesian aquifer, prevent erosion of overlying geologic materials, and confine the flow to within the casing. Flowing well discharge control shall be provided.

Michigan Construction Construction - Drilling Water

Shall be potable water that contains a free chlorine residual of not less than 10 parts per million at the time of use and shall be conveyed in containers that are clean and capable of being maintained in a clean condition. Surface water shall not be used for drilling purposes unless it is obtained from a municipal water supply system.

Michigan Construction Construction - Bedrock Well

If bedrock within 25 ft of surface, oversized boreholes drilled and grouted with neat cement to minimum depth of 25 ft. If area only where well developed in fractured, jointed or cavernous bedrock, casing may termination in formation only if there is not less than 25 ft of soil above bedrock, if no record of contamination and no connections to surface water.

Michigan Casing Coupling

Shall be recessed or reamed and drifted couplings that are manufactured in compliance with the standards of ASTM specification A 589-89a or API specification 5L-90. Shall have a design, taper, and type of thread that is consistent with the thread of the pipe and threads shall not be exposed on the pipe.

Michigan Annular Space Annular Space - General

An annular space between a permanent casing and temporary casing shall be grouted during temporary casing removal by pumping neat cement or bentonite grout, or by pouring bentonite chips, bentonite pellets, or granular bentonite, into the annular space. Granular bentonite shall not be poured into an annular space that contains drilling fluid or water.

Michigan Well Head Completion Well Head Completion - General

Above grade connection into the top or side of a well casing shall not be less than 12 inches above ground surface and shall exclude dirt and foreign matter by way of threaded or welded connection, rubber expansion sealer, bolted flanges with rubber gaskets, or a weather-tight, vermin-proof well cap.

Michigan Additional Requirements Additional Requirements - General

A driven well point shall not be used as a water supply well without the written approval of the health officer. Drilling fluids or additives that contain guar gum or other biodegradable organic materials shall not be used during the drilling of a well.

Maryland Pump Pits Pump Pits - General

Not allowed.

Maryland Well Pits or Vaults Well Pits or Vaults - General

Not allowed.

Maryland Pumps Pumps - General

Shall conform to COMAR 09.20, or the local plumbing code, whichever is more stringent. Installation must be followed by disinfection.

Maryland Grout Bentonite - Clay

Bentonite with minimum solids content of 20% may be used to fill annular space when mixed as a slurry at 2 lbs bentonite per 1 gal. water, or in chip or pellet form and if hydrated per specifications when annular space is less than 20 ft depth. May not be used in contact with groundwater with pH below 4.0 or TDS greater than 1,000 mg/L. Sand-bentonite ratio may not be greater than 250 lbs sand per 50 lbs bentonite. Sand shall be "000" well gravel that meets the following: must be 95% silica sand, have uniformity coefficient not greater than 1.7, and have particle size range of 0.60 to 0.15 mm. Inside casing may be grouted with bentonite chips or pellets if drilling fluid, if present, is thinned to viscosity of less than 30 seconds and weight is less than 9 lbs/gal.

Maryland Gravel Pack Gravel Pack - General

Must be water-washed, disinfected, and free from clay, silt, and organic material. May not connect aquifers.

Maryland Fracturing Fracturing - General

Specific regs, hydrofracturing. Shall not be conducted without approval from the Approving Authority.

Maryland Electrical Connection Electrical Connection - General

Shall conform to the National Electrical Code, or to the local electrical code, whichever is more stringent.

Maryland Upper Terminal Upper Terminal - General

All wells shall be extended at least 8 inches above final grade, 24 inches above finished grade in areas likely to flood.

Maryland Grout Thermally Enhanced Cementatious Grout

Permeability rating not less than 1x10(^-7), mixed per manufacturer specifications.

Maryland Grout Unconfined Aquifer - Hydro Area 1 & 2

For well screened in unconfined aquifer in Hydro Area 1 & 2, annular space shall be grouted to depth of at least 20 ft.

Maryland Grout Grout Height

If pump not installed, final grout height shall be at ground grade, if pump is to be installed, final grout height shall be immediately below pitless adapter or unit. All grout emplaced in one continuous operation upwards from bottom of casing or specified depth.

Maryland Grout Confined Aquifer - Hydro Area 2

Wells in Hydro Area 2, depth of grouting may not be less than 30 ft. The annular space above the screen and below the grout shall be completely filled with clay, drill cuttings, or sand before grouting operations begin. For wells intended to have a yield of less than 5,000 gpd, the grout shall extend from a minimum of 5 ft into the first confining bed, which is at least 5 ft thick, to the land surface.

Maryland Disinfection Disinfection - Nonresponsive Wells

If unresponsive after repeating disinfection procedure, water with chlorine concentration of 100 mg/l and water at least two times the volume of chlorinated water, but not less than 50 gallons, shall be introduced into the well in order to completely displace the volume of chlorinated, standing water and force it out into the water bearing formation. If bacteriological contamination persists, Approving Authority may require well to be sealed and abandoned.

Maryland Construction Construction - Water

Use water from best possible source; public water supply, potable water supply, nonpotable or temporary well constructed for purposes of construction but must not have turbidity of more than 25 standards units, except when turbidity is due to oxidation of dissolved iron or manganese. Must have a color of not more than 25 standard units, and contain no objectionable odor. Chlorine compound added to water from public supply shall produce a free chlorine residual of at least 1.0 mg/L, potable supply shall have chlorine residual of at least 3.0 mg/L, and nonpotable supply shall be dosed with chlorine to produce minimum free chlorine residual of 50 mg/L. Driven wells may not be used for a potable water supply.

Maryland Annular Space Annular Space - General

Minimum of 1 1/2 inches.

Massachusetts Construction Dug Well - General

Wall thickness of precast concrete pipe should be minimum 3 inches thick and poured-in-place concrete at least 6 inches thick. Casing should be watertight to depth of production aquifer or depth of 20 ft. below preexisting ground surface, whichever is greater. Upper terminus of casing should extend 12 inches above pre-existing ground surface or 2 ft. above highest recorded flood, whichever is greater. With precast concrete pipe, annular space should be at least 4 inches larger than O.D. casing and should e sealed with neat cement grout or sand cement ground to depth below frost line. Intake should be perforated casing or loosely laid wall of stone, concrete block, or brick. Must be of strength to withstand pressures and seated to prevent settling.

Massachusetts Construction Dug Well - Buried Slab

Steel reinforced concrete slab should be at least 3 inches thick and placed at least 20 ft. below pre-existing ground surface. Upper casing should be minimum 4 inches diameter and either firmly imbedded in slab, or connected to pipe cast in slab. Seal between upper casing and slab should be watertight and annular space grouted with neat cement grout or sand cement grout. If pump installed over well, a concrete platform should be equipped with pipe sleeve cast-in-place and sloped away from sleeve.

Massachusetts Construction Dug Well - Standard

Should have cover of reinforced concrete at least 4 inches thick, should rest on and overlap outer edge of well casing by 2 inches and should be constructed without joints. Pump sleeve sized to accommodate pump type and pump piping should be cast in place when cover fabricated. Top of cover should be sloped to drain away from sleeve. If manhole installed, should have curb extending at least 2 inches above top of cover and cast in place with cover fabrication. Manhole should have overlapping cover with sides extending downward around curb at least 1 &1/2 inches. Joint between curbing and cover should be watertight with plastic sealing compound and manhole cover locked or bolted in place.

Massachusetts Construction Non-Flowing Artesian Well

Extend oversized drill hole into but not through confining layer, ensure no water flows from artesian zone into drill hole. Install an unperforated, watertight protective casing with drive shoe. Extend drill hole into artesian zone after grout completely set, ensure using packer that provides watertight seal. Should be equipped with shutoff valve and backflow preventer.

Massachusetts Pumps Pumps - Installation

All pumps should be installed either below the frost line with a pitless adapter or in some other heated and protected sanitary location. Above-ground pumps should be installed in sheltered, dry, accessible locations and should be protected from freezing. Shallow-well pumps should be installed as near the well or water source as possible to minimize suction lift. Submersible and helical rotor pumps must be installed in the well. A deep well jet pump may be offset from the well.

Massachusetts Casing Concrete

Only to be used in construction of dug wells. Should be at least 3 inches thick, meet or exceed ASTM C67 Class III specifications, be free of blemishes that could impart integrity of the well.
