Kentucky Construction Construction - General

Annular space shall be filled with neat cement-bentonite slurry, bentonite, native clay, or drill cuttings. Casing to be set into bedrock a min of 10 ft. if in consolidated formations when overburden is < 30 ft. thick. Casing to be set into bedrock a min of 2 ft. if in consolidated formations when overburden is > 30 ft. thick. Casing to extend a min of 2 ft. into stable rock below the lowermost void whenever voids are encountered. Coupling to be cemented to bottom of casing. Kentucky Abandonment Abandonment - General

At least the upper 5 ft. of casing, liner pipe, brick, stone, & metal to be removed. Driller shall fill the well with sealing materials or inert earth materials from bottom of the well to a minimum of 20 ft below ground surface, and fill remainder with sealing materials to minimum of 5 ft below ground surface. Uppermost 5 ft of well shall be filled with sealing materials or other suitable, inert earth material. Massachusetts Caps Caps - General

Any well, except a dug well, that does not terminate in the base of a pump should be equipped with a sanitary seal or watertight well cap designed to prevent surface water and foreign matter from entering the well. A flowing artesian well should be equipped with a shut-off valve and a backflow preventer so that the flow of water can be stopped completely when the well is not in use.. Temporary covered should be installed during interruptions of 1 week or longer. Semi-permanent watertight cover should be installed. For steel-cased well, semi-perm covered of steel or cast iron cap of at least 3/16 inches thick should be tack welded to top of casing.

Massachusetts Access Ports & Tubes Access Ports & Tubes - General

All new wells (except dug wells) and wells repaired or altered shall be equipped with access port or pressure gage fitting to include a control valve. Access ports should have inside diameter of at least 1/2 inch and fitted with threaded plug or cap. Airlines and removable well caps are acceptable as access ports. When a spool type of pitless adapter is used which prevents the casing from having an unobstructed opening to the water surface, a 3/4 inch diameter pipe may be attached to the spool and extended to the surface below the well cap.

Massachusetts Abandonment Abandonment - General

Casing to be cut off before seal is placed. Materials not to be poured. Materials to be introduced into bottom of well & proceed up to about 4 ft. below ground surface.
