Minimum standard of 1.5 inches of cement grout poured in the space between the borehole wall and the casing in one continuous application from a minimum depth of 20 feet to the land surface.

Shall have watertight curbing extending at minimum 1 ft. above surface down to static water level, or into confining formation if well is artesian. Curbing shall consist of poured cement grout with a minimum thickness of 6 in., or casing with cement grout within a minimum of 3 in. annular space, concrete block with grout, or rock with cement grout may also be used. Must be sealed at the surface with a watertight, tamper-resistant cover.

Well casing shall extend into the confining formation immediately overlying the artesian aquifer and shall be grouted from a minimum of 10 ft. into the confining formation to the land surface. If the well flows at land surface, it shall be equipped with a control valve, or means of completely controlling the flow, and be available for inspection at all times. If leaks occur adjacent to the well or around the well casing, within 30 days the well shall be completed with the seals, packers, or casing and grouting necessary to eliminate such leakage or the well shall be abandoned according to R12-15-816.

Abandoned wells in consolidated formations shall be plugged for 50ft below the bottom of casing to a minimum depth of 100 ft below land surface, and remaining depth to be filled with clean sand or pea gravel. If more than one major aquifer, must have minimum seal of 50ft within each confining unit between aquifers, and between aquifer and surface. Wells in unconsolidated formations to be filled with sand or natural material to 12 ft. of surface, then filled with bentonite or cement from 12ft to 2ft. Abandoned well with no formation information is to be filled from bottom to 2 ft. of top with cement. Should be reported by the water well contractor to the AWWCC on a well abandonment form to be approved by the AWWCC within ninety (90) days of abandonment.
